About this role

A Community Outpatient Pharmacist provides pharmaceutical care for outpatient populations according to professional standards and statutory requirements. Directs functional activities of outpatient pharmacy technicians, students, and interns. Ensures high quality, safe, and cost-effective drug therapy through clinical support and medication preparation.

Provides education, consultations, immunizations, and medication therapy management to improve patient outcomes. Interprets and evaluates prescribers' medication orders for appropriate therapy per legal standards. Reviews patient profiles for allergies, interactions, contraindications, and dosing issues.

Communicates with physicians, nurses, and healthcare providers to clarify orders and recommend alternatives, documenting interventions. Performs medication histories and counseling, supplying education materials as needed. Works with diverse healthcare team members and patient populations.

Engages in professional activities to enhance public health awareness. Administers immunizations and maintains knowledge of applicable laws and regulations. Collaborates in conjunction with other healthcare team members for optimal patient care.

Requirements

  • Bachelor’s degree in Pharmacy from an accredited College of Pharmacy
  • Doctor of Pharmacy degree (PharmD) strongly preferred
  • Broad knowledge of medications, compounding, and dispensing
  • Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics
  • Interpersonal skills to communicate effectively with hospital personnel and diverse patients
  • Training and skill to administer immunizations
  • General understanding of computers, including operations and data retrieval
  • Maintain working knowledge of applicable Federal, State, and Local laws and regulations

Responsibilities

  • Interprets and evaluates all prescriber’s medication orders to ensure appropriate drug therapy
  • Reviews, maintains, and screens patient medication orders and profiles for allergies, drug interactions, contraindications, and dosing issues
  • Communicates with physicians, nurses, and healthcare providers to intervene on unclear or inappropriate orders and recommends alternative therapy
  • Performs patient medication histories and counseling when necessary or requested, providing education materials
  • Consults with and advises physicians and other health care professionals
  • Provides clinical support, medication preparation and distribution
  • Directs functional activities of outpatient pharmacy technicians, students, and interns
  • Administers immunizations and provides medication therapy management
