The Clinical Pharmacist provides comprehensive pharmaceutical care through clinical services, formulary management, and integrated drug distribution. This role requires availability to work day shift, Monday - Friday, as needed, and includes rounding with interdisciplinary teams and documenting clinical interventions.

You will serve as a preceptor for pharmacy students and residents, assist with investigational drug studies, and help develop drug protocols. Additionally, you will participate on the Pharmacy and Therapeutics Committee and support regulatory compliance with agencies like The Joint Commission.

Requirements

  • Active RPH - Registered Pharmacist license
  • Minimum 3 years of clinical pharmacy experience or completion of a PGY1 Pharmacy Residency
  • Board Certification (e.g., BCPS, BCGP) strongly preferred within 180 days
  • Bachelor's in Pharmacy or Doctorate in Pharmacy (PharmD) preferred
  • Availability to work day shift, Monday - Friday, as needed

Responsibilities

  • Provide recommendations and/or manage therapy associated with antimicrobial stewardship, pharmacokinetics, anticoagulation, parenteral nutrition, or other medication regimens as prescribed by the provider
  • Document clinical interventions on an ongoing basis
  • Round with interdisciplinary teams, as able
  • Assist in investigational drug studies when needed
  • Perform medication history review and assist providers with medication reconciliation as needed
  • Develop and provide educational programs and inservices for healthcare practitioners and patients
  • Serve as a preceptor for pharmacy students and for the Pharmacy Residency Program
  • Participate on the Pharmacy and Therapeutics Committee and assist with Medication Use Evaluations and formulary maintenance
