Requirements

• Graduate of an accredited Pharmacy program (Required).
• Active Florida Pharmacist License (PS LICENSE).
• Relevant pharmacy work experience or training preferred.
• Critical thinking skills and decisive judgment.
• Effective verbal and written communication skills.
• Ability to work well with others in a team with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.

Responsibilities

• Manage and recommend drug therapy regimens as part of an interdisciplinary team to provide patient and family centered care.
• Review, interpret and evaluate medication orders, labs and other clinically relevant information to ensure safe and optimal therapeutic outcomes and document interventions appropriately.
• Provide clinical pharmacy services including renal/hepatic dosage adjustment, pharmacokinetic dosing, anticoagulation monitoring, total parenteral nutrition, and antimicrobial stewardship.
• Supervise and delegate tasks to technicians and other support staff to ensure timely prescription/order verification, preparation, and distribution of medications, compounding and/or supplies.
• Provide drug information to patients and healthcare providers including medical, nursing, and pharmacy staff.
• Coordinate workflow as needed to optimize pharmacy operations and patient care.
