About this role
Coordinates and provides clinical pharmacy services to patients, working collaboratively with the interdisciplinary team to ensure patient and family centered care. Manages and recommends drug therapy regimens as part of this team. Reviews, interprets and evaluates medication orders and clinically relevant information to ensure safe and optimal therapeutic outcomes.
Manages drug therapy and provides clinical services including renal/hepatic dosage adjustment, pharmacokinetic dosing, anticoagulation monitoring, total parenteral nutrition, and antimicrobial stewardship. Documents interventions appropriately and coordinates workflow with pharmacy technicians to ensure timely medication distribution.

Requirements
- Graduate of an accredited Pharmacy program (Required).
- Active Florida Pharmacist License (PS LICENSE).
- Relevant pharmacy work experience or training preferred.
- Critical thinking skills and decisive judgment.
- Effective verbal and written communication skills.
- Ability to work well with others in a team with minimal supervision.
- Ability to work in a stressful environment and take appropriate action.
Responsibilities
- Manage and recommend drug therapy regimens as part of an interdisciplinary team to provide patient and family centered care.
- Review, interpret and evaluate medication orders, labs and other clinically relevant information to ensure safe and optimal therapeutic outcomes and document interventions appropriately.
- Provide clinical pharmacy services including renal/hepatic dosage adjustment, pharmacokinetic dosing, anticoagulation monitoring, total parenteral nutrition, and antimicrobial stewardship.
- Supervise and delegate tasks to technicians and other support staff to ensure timely prescription/order verification, preparation, and distribution of medications, compounding and/or supplies.
- Provide drug information to patients and healthcare providers including medical, nursing, and pharmacy staff.
- Coordinate workflow as needed to optimize pharmacy operations and patient care.
