The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to patients in accordance with Option Care policies and applicable regulatory standards.

This entry-level role focuses on clinical pharmacy practice in the alternate-site setting. The pharmacist coordinates patient services with the Nursing Department, monitors progress toward defined outcome goals, and educates patients, caregivers, and other professionals on appropriate medication use.

Requirements

  • Current pharmacist license in good standing
  • Knowledge of pharmacy practice professional standards and applicable regulatory entities
  • Competence in pharmacokinetic principles and formulas
  • Ability to assess patient laboratory values and nutritional status
  • Skill in designing and evaluating patient-specific pharmacy care plans

Responsibilities

  • Determines the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing medical condition and prescribed therapy.
  • Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
  • Makes suitable recommendations for modifications in prescribed medication therapy or monitoring parameters based on collected patient information.
  • Modifies pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information, including pharmacokinetic principles and laboratory values.
  • Provides medication-use education to Option Care staff, other professionals, home care patients, and caregivers using effective counseling techniques.
  • Ensures continuity of pharmaceutical care to and from the home and other patient-care settings through systematic communication procedures.
  • Prepares and dispenses medications using appropriate techniques while following Option Care policies and procedures and accurately calibrating equipment.
