Functions in all sections of the Pharmacy under minimal supervision.
Performs decentralized activities in ED/ICU and central pharmacy with 10-hour shifts and 8-hour shifts every third weekend. Prepares drugs for distribution, provides drug information and clinical pharmacy services, maintains drug control. Provides work direction to certified and non-certified Pharmacy Technicians.
Reviews medication orders for completeness and appropriateness, monitors patient therapy for interactions, allergies, and incompatibilities, making recommendations to physicians. Documents interventions in patient records and pharmacy profiles, enters data into the pharmacy system. Participates in pharmacokinetic dosing service and provides services to operating room, PACU, and same day surgery.
Compounds pharmaceuticals, fills prescriptions for outpatients and employees, maintains security of controlled substances. Conducts medication counseling, inservice education, and reports medication errors. Requirements
- Licensed as a Registered Pharmacist
- Ability to function under minimal supervision in all pharmacy sections
- Knowledge of drug interactions, allergies, incompatibilities, and patient age-specific needs
- Proficiency in pharmacy data processing systems
- Experience directing work of certified and non-certified Pharmacy Technicians
- Compliance with all relevant policies, procedures, guidelines, and regulatory standards
- Ability to compound sterile parenteral medications and specialty items
Responsibilities
- Reviews all medication orders verifying the completeness and appropriateness of drug therapy
- Monitors patient’s total drug therapy checking for drug interactions, allergies, incompatibilities and appropriateness
- Documents all medication clarification/interventions in the patient’s medical record and/or pharmacy profile
- Enters medication data into the pharmacy data processing system assuring data is current and accurate
- Provides information to physicians and hospital staff on new drugs, warnings, incompatibilities and contraindications
- Participates in the Pharmacy’s pharmacokinetic dosing service in consultation with the clinical pharmacist
- Compounds various pharmaceuticals and prepares sterile parenteral medications
- Maintains the security of controlled substances and perpetual inventory of Schedule II drugs
