About this role

The Pharmaceutical Care Pharmacist provides safe and effective medication therapy for optimal patient outcomes. Ensures completion and documentation of all work including that of technicians. Requires population specific competencies and adherence to National Patient Safety Goals based on patient contact level.

Advises on medications, medication therapy, pharmacokinetic recommendations, nutritional information, therapeutic drug monitoring, and pain management. Verifies physician orders for safe optimal therapy, checking indications, allergies, dosing, interactions, duplications, cost-effectiveness, and formulary compliance. Accountable for accuracy of all technician work within expected turnaround time.

Ensures healthcare areas have only properly dispensed and stored medications. Maintains readiness of emergency carts by filling or assigning to technicians. Compounds and dispenses pharmaceutical products accurately prepared, labeled, documented, and distributed.

Reports to the Pharmacy Supervisor. Position may involve physical demands and respiratory fit test requirements; consult Physical Demands Project SharePoint or Risk Management. Hazards from chemotherapeutics and antivirals minimal with safety precautions.

Requirements

  • Bachelor's degree in Pharmacy
  • Current Oklahoma Pharmacy license
  • 15 hours of approved continued education annually
  • Documented skills including Pharmacokinetics, Nutrition Support, Pain Management, ADR reporting, Drug Use Evaluation activities
  • Ability to communicate effectively in English

Responsibilities

  • Advise about medications, medication therapy, pharmacokinetic recommendations, nutritional information, therapeutic drug monitoring and pain management
  • Verify physician orders for safe optimal medication therapy, indications, allergies, dosing, interactions, duplications, cost-effectiveness and formulary compliance
  • Verify, initial, and be accountable for the completion and accuracy of all work including that of technicians within expected turnaround time
  • Ensure all healthcare areas only have medications that are properly dispensed and stored
  • Ensure readiness of all emergency carts by filling them or assigning to technicians
  • Compound and dispense all pharmaceutical products, ensuring accurate preparation, labeling, documentation, and distribution
