About this role

The Clinical Pharmacist Float provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes for patients. This role actively participates in multidisciplinary patient care rounds, medication order evaluation and verification, and drug distribution support. Clinical services include medication histories, pharmacokinetic dosing, drug information requests, and patient counseling.

Daily work centers on reviewing medication orders for accuracy, appropriateness, and efficacy while monitoring drug therapy regimens. The pharmacist delivers pharmacokinetic consultations, prevents and reports adverse drug reactions, and manages therapy during medical emergencies such as code blue. Admission and discharge medication reviews further support safe patient transitions.

Within UofL Health's integrated academic system, the float pharmacist collaborates with physicians, nurses, and other clinicians across multiple sites. This environment supports implementation of disease management programs, formulary management through therapeutic interchanges, and compliance with established policies and procedures.

The position includes opportunities to develop new clinical programs and services while advancing professional expertise. Participation in scholarly activities strengthens both individual practice and system-wide pharmacotherapy standards for all patients.

Requirements

  • Doctor of Pharmacy (PharmD) degree
  • Active pharmacist license in Kentucky
  • PGY1 residency training or equivalent clinical experience
  • Demonstrated expertise in pharmacotherapy and clinical interventions
  • Ability to serve as subject matter expert for formulary and policy compliance

Responsibilities

  • Reviews medication orders for accuracy, appropriateness and efficacy
  • Actively participates in multidisciplinary patient care rounds
  • Provides pharmacokinetic consultations and dosing recommendations
  • Documents all pharmacy interventions in patient electronic medical record
  • Monitors drug therapy to evaluate appropriateness of use, dose, and drug interactions
  • Prevents, detects, monitors, documents, and reports adverse drug reactions and medication errors
  • Participates in the drug therapy management of medical emergencies
  • Performs admission history and discharge medication review and counseling
