About this role

Join our growing Retail team as a part-time Retail Pharmacist for a 3 days a week schedule. Provide comprehensive pharmaceutical care ensuring safe, effective, and cost-conscious use of pharmaceuticals. Demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities.

Assure timely medication order processing by participating in order entry, order verification, and reviewing orders for resolution. Complete patient-specific medication profile reviews resulting in protocol-based drug therapy including pharmacokinetic dosing and IV to PO conversions. Adjust doses as appropriate for parameters such as renal or hepatic dysfunction.

Actively participate on the multidisciplinary team by attending patient care rounds and monitoring drug therapy for safe, cost-conscious regimens. Support department staffing needs by flexing to other areas or alternative scheduling. Participate in medication reconciliation at admission, transfer, and discharge with patient education.

Continually report medication incidents and adverse drug reactions in Patient Safety Net and engage in review activities. Share pharmaceutical expertise through educational presentations for pharmacy and non-pharmacy staff. Support pharmacy residency programs, students, and interns by precepting rotations and providing mentorship.

Requirements

  • Bachelors Degree in Pharmacy from an accredited college or university
  • Licensed Pharmacist - State Board of Pharmacy in state of primary practice site
  • Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) certification as necessary
  • Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
  • Completion of an accredited pharmacy post graduate training program (PGY-1 and/or PGY-2 residency program) preferred
  • Board of Pharmaceutical Specialties certification in chosen area of specialty preferred

Responsibilities

  • Actively participates in organizational and departmental performance improvement efforts to strengthen systems, processes, policies and procedures
  • Participates on the multidisciplinary team by attending patient care rounds and providing patient-focused pharmaceutical care
  • Supports department staffing needs by flexing to other areas or alternative scheduling for coverage
  • Assures timely medication order processing by participating in order entry, verification, and reviewing orders
  • Completes patient-specific medication profile reviews and executes protocol-based drug therapy with documentation
  • Reports medication incidents and adverse drug reactions in Patient Safety Net and participates in reviews
  • Participates in medication reconciliation at admission, transfer, and discharge and provides patient education
  • Supports pharmacy residency programs, clerkship students and interns by precepting rotations and providing mentorship
