Equitas Health, established in 1984, is a regional not-for-profit community-based healthcare system serving over 67,000 individuals in Ohio, Kentucky, and West Virginia. It focuses on primary care, retail pharmacy, and HIV/STI prevention for the LGBTQ+ community. The Clinical Community Pharmacist delivers outstanding pharmaceutical care using knowledge of pharmaceuticals and pharmacy standards.
Daily tasks include verifying prescription accuracy for safety and optimal outcomes, co-managing pharmacy technicians, and handling inventory management. Pharmacists check prescriptions for clinical appropriateness, provide patient consultations on regimens, side effects, and allergies. They also facilitate the 340B program and formulary management.
Collaboration occurs with medical center providers, case managers, behavioral health therapists, and supporting staff to optimize health outcomes and medication adherence. The role involves working with pharmacy students and marketing services to medical offices and patients. Team efforts promote Equitas Health Pharmacy in HIV care excellence.
Growth opportunities include building in-pharmacy clinical services like adherence programs, specialty disease management, and immunizations in a cost-effective manner. Pharmacists present to groups, train staff, and contribute to financial success. The position supports community health initiatives and patient assistance services.
Requirements
- Traveling, driving or having reliable transportation

Responsibilities
- Provide superior customer service to patients, providers, community members, other employees and referral sources
- Facilitate management of the pharmacy with the Pharmacy Manager, including co-management of technicians, 340B program, formulary, prescription checks, and inventory
- Collaborate with Equitas Health Medical Center providers and staff to optimize health outcomes for clinic patients
- Provide consultation to patients on therapeutic regimens, administration, side effects, and allergic precautions
- Deliver specialized pharmaceutical therapy and services, including adherence programs, specialty disease management, and immunization services
- Collaborate with case managers, behavioral health therapists, and supporting staff to ensure medication adherence
- Provide guidance on patient assistance services to ensure access to prescribed medications
- Market pharmacy dispensing and clinical services to medical offices and patients within the service area
