About this role
The Pharmacist performs medication management activities to ensure safe, rational, and cost-effective therapy while maintaining compliance with all applicable rules, regulations, and standards. Maintains established departmental policies and procedures and complies with department objectives. This role supports the pharmacy department at St. Joseph Hospital.
Process orders and provide medication review for appropriateness, safety, and efficacy during order processing. Ensure accurate preparation of compounded and non-compounded products with safe handling consistent with regulations. Oversee distributive functions to facilitate accurate and timely medication availability to patients and staff.
Oversee pharmacy technicians and support personnel activities based on department need. Work collaboratively to communicate verbally and in writing with pharmacy team members, healthcare providers, patients, caregivers, and leaders. Maintain responsibility and accountability for the security of pharmacy inventory.
Identify, document, and resolve potential and actual adverse medication events, evaluating reports for trending and prevention. Perform medication education to improve patient understanding and adherence. Participate in departmental medication safety, quality assurance, drug utilization reviews, and student precepting.
Requirements
- Maintains current license for pharmacist in the state(s) where practicing
- Valid state issued ID
- Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD
- Experience in field of pharmacy practice
- Exhibit proficient use of all computer hardware and software
- Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served
- BLS Basic Life Support (preferred)
- Immunization Certified (preferred)
