Equitas Health, established in 1984, is a regional not-for-profit community-based healthcare system serving over 67,000 individuals in Ohio, Kentucky, and West Virginia. It focuses on primary care, retail pharmacy, behavioral health, and HIV/STI prevention for the LGBTQ+ community. The Clinical Community Pharmacist delivers outstanding pharmaceutical care using working knowledge of pharmaceuticals and pharmacy practice standards.
Daily tasks include verifying prescription accuracy for safety and optimal patient outcomes, providing superior customer service to patients and providers, and facilitating pharmacy management with the Pharmacy Manager. Responsibilities cover inventory management, checking prescriptions for clinical appropriateness, and input into the 340B program and formulary. The role requires traveling, driving reliable transportation, and utilizing computers for processing and research.
Collaborate closely with Equitas Health Medical Center providers, case managers, behavioral health therapists, and pharmacy technicians to optimize health outcomes and ensure medication adherence. Work with the pharmacist team and other departments to build in-pharmacy clinical services like adherence programs, specialty disease management, and immunizations. Promote pharmacy services to medical offices, patients, and pharmacy students for long-term relationships.
Present to small and large groups, market dispensing and clinical services, and promote Equitas Health Pharmacy as a collaborator in HIV care excellence. Supervise and train staff while adhering to all state and federal pharmacy laws. Opportunities include implementing services for high patient satisfaction and maximal financial success.
Requirements

Responsibilities
- Provide superior customer service to patients, providers, community members, other employees and referral sources
- Facilitate management of the pharmacy with the Pharmacy Manager including co-management of pharmacy technicians, 340B program, formulary, prescription checks, inventory management
- Collaborate with Equitas Health Medical Center providers and staff to optimize health outcomes for medical clinic patients
- Provide consultation to patients to ensure understanding of therapeutic regimens, administration, side effects, and allergic precautions
- Deliver specialized pharmaceutical therapy and services including adherence programs, specialty disease management, and immunization services
- Collaborate with case managers, behavioral health therapists, and supporting staff to ensure access to and adherence to medication regimens
- Provide guidance on patient assistance services to ensure access to prescribed medications
- Market pharmacy dispensing and clinical services to medical offices and patients within the service area
