About this role

Passionate about contributing to DMC’s service development across our primary care team, the Clinical Pharmacist enables involvement in decisions that affect care delivery. We seek enthusiastic and motivated pharmacists to join the team and develop services as keen learners focused on clinical development.

The role centres on providing expert pharmaceutical care to achieve clinical excellence and improved patient outcomes. Primary duties include ensuring safe and effective medication use while collaborating with healthcare professionals in a patient-facing capacity within a multi-disciplinary team.

Daily work involves proactively managing patients with co-morbidities and supporting general practice staff on prescription and medication queries. The post holder handles repeat prescriptions, acute requests, medicines reconciliation on transfer of care, and clinical triage of the appointment book.

Opportunities exist to provide leadership on medicines optimisation and quality improvement while managing aspects of the quality and outcomes framework. The position supports service development across the organisation and offers scope for clinical growth within primary care.

Requirements

  • Master’s degree in pharmacy, MPharm, B.Sc. or BPharm or equivalent at time of qualification.
  • GPhC Registered.
  • Independent Prescriber.
  • Maintain registration as a pharmacist with the General Pharmaceutical Council and act in accordance with the code of ethics.

Responsibilities

  • Proactively manage patients with co-morbidities in a patient-facing role.
  • Support general practice staff with prescription and medication queries, repeat prescriptions, acute requests, and medicines reconciliation.
  • Provide leadership on medicines optimisation and quality improvement while managing aspects of the quality and outcomes framework.
  • Partake in clinical triage and management of the GP practices appointment book.
  • Develop and manage a medicines management plan and deliver patient services per practice policy and guidance.
  • Perform audits including prescription chart screening for clinical appropriateness and undertake medicines management audits.
  • Provide clinical training seminars to Health Care Professionals and work alongside them to deliver patient-centred services.
