About this role

St. Croix Health is seeking a part-time (0.6) clinical Pharmacist to float to all Pharmacy locations including St. Croix Falls, Frederic, and Balsam Lake. This role ensures the safe and effective use of medications while providing high-quality pharmaceutical care. It promotes wellness and supports optimal patient outcomes in collaboration with healthcare professionals.

The Pharmacist dispenses medications accurately according to legal and organizational standards, consults patients on proper use, side effects, and adherence. Reviews prescriptions and profiles to identify and resolve medication-related problems, monitors errors and reactions. Compounds customized medications and manages inventory to maintain adequate stock.

Works closely with physicians, nurses, and other providers to optimize care plans and medication therapy. Educates patients on chronic conditions, lifestyle changes, and preventative strategies while providing vaccination services. Ensures regulatory compliance including controlled substances laws and maintains accurate records.

Requirements

  • Graduate of a college of pharmacy accredited by the American Council for Pharmaceutical Education
  • Current and valid Wisconsin Pharmacist license
  • BLS certification required
  • Immunization certification preferred within 6 months of hire
  • Prior Pharmacist experience preferred

Responsibilities

  • Accurately prepare and dispense prescription medications according to legal, regulatory and organizational standards
  • Provide advice and information to patients regarding proper medication use, side effects, potential interactions and adherence
  • Review patient prescriptions and medication profiles to ensure safe and effective medication therapy and resolve problems
  • Monitor for medication errors and adverse drug reactions and document accordingly
  • Prepare customized medications for patients following prescriber instructions and standards
  • Collaborate with physicians, nurses, and other healthcare providers to optimize patient care plans
  • Monitor and manage medication inventory to ensure adequate stock and proper storage
  • Educate patients about managing chronic conditions, lifestyle changes, preventative strategies and provide vaccination services
