About this role
This clinical pharmacist role with Tri-Cities Community Health combines direct patient care under a Collaborative Drug Therapy Agreement with traditional dispensing responsibilities in a community health setting. You will manage medication therapy for chronic disease states, perform medication reconciliation, and counsel patients on their prescriptions.
Day-to-day duties include initiating, modifying, or discontinuing medications per established protocols, addressing prior authorizations and refill requests, and maintaining accurate documentation in both the electronic health record and pharmacy software system. You will also complete final prescription checks and review pharmacy profiles per Washington State requirements.
You will collaborate closely with providers on drug therapy matters, supervise and train pharmacy technicians, and provide in-service education to clinic staff. The role requires working within a team concept while assuming final responsibility for all work performed by support staff.
This position offers the opportunity to grow clinically by working under TCCH protocols—including therapeutic interchange and refill protocols—while participating in peer reviews and quality improvement initiatives. You will help shape the pharmacy's clinical services in a mission-driven organization.
Requirements
- Ability to perform the duties of a clinical pharmacist including medication management and patient counseling.
- Familiarity with Collaborative Drug Therapy Agreements to initiate, modify, or discontinue medications for chronic disease states.
- Competence in patient counseling, medication reconciliation, and clinical recommendations regarding medication therapy.
- Proficiency with electronic health record documentation and pharmacy software systems.
- Knowledge of Washington State pharmacy requirements and regulations.
- Experience supervising pharmacy technicians and ancillary staff.
- Understanding of TCCH protocols including refill and therapeutic interchange protocols.
- Ability to participate in peer reviews and quality improvement initiatives.
Responsibilities

- Assist providers in matters concerning drug therapy as requested.
- Maintain accurate documentation in both the electronic health record and pharmacy software system.
- Complete final check of prescriptions and review pharmacy profiles per Washington State requirements.
