The Population Health Pharmacist will help communities thrive as a key member of the Adhere Operations Team. Work closely with Stellus Rx leaders and across the organization to unlock the health of millions by turning 'use as prescribed' into a guarantee. Leverage pharmaceutical expertise, data analysis, and managerial skills to optimize medication therapies and enhance patient adherence.

Develop, implement, and evaluate population health initiatives centered around medication management. Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention. Collaborate with multidisciplinary teams including physicians, nurses, social workers, and other healthcare professionals to coordinate care plans.

Participate in quality improvement projects to enhance medication-related outcomes and promote evidence-based practices. Provide leadership within the pharmacy team and mentorship to junior staff. Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Active pharmacist licensure in the state or region of practice
  • Advanced training or certification in population health management or a related field (preferred)
  • Strong analytical and data interpretation skills
  • Experience in pharmacy practice, preferably in a population health or managed care setting
  • Excellent communication, collaboration, and leadership abilities
  • Knowledge of healthcare regulations, policies, and quality improvement methodologies
  • Fluent in Spanish (preferred)

Responsibilities

  • Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes
  • Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related outcomes
  • Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering population-specific needs
  • Design and implement programs to educate patients about their medications, ensuring understanding, adherence, and management of potential side effects
  • Work closely with healthcare teams, including physicians, nurses, social workers, to coordinate care plans and interventions
  • Participate in quality improvement projects aimed at enhancing medication-related outcomes and reducing adverse events
  • Stay updated with pharmaceutical regulations, ensuring compliance with relevant laws, policies, and guidelines
  • Provide leadership within the pharmacy team, mentorship to junior staff, and contribute to departmental goals
