St. Croix Health is seeking a casual, patient-focused clinical Pharmacist to float across all pharmacy locations in St. Croix Falls, WI, Frederic, WI, and Balsam Lake, WI. Casual roles require working two shifts per month. The Pharmacist reviews, prepares, and dispenses medications ensuring compliance with best practices, policies, procedures, standards, and regulations.

The role involves utilizing clinical knowledge to provide safe, effective, and affordable medications while working closely with the medical team. Services include medication therapy management (MTM), immunizations for community members, and durable medical equipment (DME). Pharmacists collaborate with healthcare professionals to monitor drug regimens and evaluate effectiveness.

Day-to-day duties encompass prescription preparedness, patient education on interactions and side effects, order reviews for allergies and duplications, and medication error monitoring. Maintain department workflow by directing support staff and listening to technician feedback. Enforce pharmacy laws, review controlled substance policies, and investigate narcotic discrepancies.

Join a team focused on patient safety and quality care in multiple retail pharmacy settings. Provide specialized services for managing conditions like diabetes, asthma, high blood pressure, and smoking cessation. Contribute to analyzing prescribing trends to ensure patient compliance and prevent harmful interactions.

Requirements

  • Graduate of an ACPE accredited School of Pharmacy with a Pharmacy degree
  • Current Wisconsin Pharmacist License required
  • Patient-focused with clinical knowledge to review, prepare, and dispense medications
  • Ability to float and work in multiple pharmacy locations
  • Knowledge of controlled substance policies and standard workflows
  • Proficiency in reviewing patient profiles for allergies, duplications, and drug interactions
  • Experience assessing medication identity, strength, purity, and suitability

Responsibilities

  • Interpret, prepare, and dispense drug orders per provider request according to policies, procedures, and protocols
  • Review prescription orders for accuracy, needed ingredients, suitability, and proper dosage
  • Assess identity, strength, and purity of medications and plan procedures for mixing, packaging, and labeling
  • Collaborate with healthcare professionals to monitor, review, or evaluate drug regimens and provide advice
  • Provide patient education on drug interactions, side effects, dosage, proper storage, and over-the-counter selections
  • Review orders and patient profiles for diagnosis, allergies, duplications, and interactions
  • Monitor for medication errors, adverse drug reactions, and document accordingly
  • Maintain department workflow, direct support staff, and comply with all pharmacy laws and standards
