Provides pharmaceutical care in coordination with other health care professionals to deliver safe, appropriate and cost effective drug therapy to patients. This role ensures optimal medication management in a community setting. Focus remains on patient-centered outcomes.

Coordinates with physicians and providers to review prescriptions and recommend therapies. Monitors drug therapy for efficacy and safety in outpatient practice. Manages cost-effective dispensing while adhering to protocols.

Requirements

B.S. in pharmacy with 2 years work experience or Doctor of Pharmacy (PharmD) degree
Eligible for licensure by the Florida Board of Pharmacy
BLS certification required within 3 months of hire
Advanced degree (MS, PharmD) or residency preferred
Experience in inpatient or outpatient pharmacy practice preferred
Board Certification by the Board of Pharmaceutical Specialties preferred
Previous experience in applicable specialty practice area preferred

Responsibilities

Provide pharmaceutical care to patients
Coordinate with other health care professionals
Deliver safe, appropriate and cost effective drug therapy
Assess patient medication needs in community pharmacy
Dispense prescriptions accurately in outpatient setting
Counsel patients on proper medication use
Monitor therapy outcomes and adjust as needed
Collaborate on interdisciplinary care plans
