About this role

Provides patient care through direct operational and clinical patient services. Ensures the safe and appropriate use of medications through drug information, drug therapy monitoring, distributive and supervisory pharmacy services within departmental policy and state/federal laws. Requires regular communication with prescribers, office staff, patients/caregivers, and healthcare professionals.

Provides accurate medication order-entry, drug distribution, drug administration, diagnostic testing, and patient care. Confirms the appropriateness of drug therapy for specialized patient populations including neonates, pediatrics, adolescents, adults, and geriatrics. Seeks to minimize turn-around-time for products and services.

Performs pharmacist duties including patient education, care management, prescription reviews for safety and efficacy, prior authorizations, and creating individual care plans. Oversees pharmacy support personnel, verifies tasks, and may function as lead or charge pharmacist. Acts as liaison between pharmacy and patient care areas to solve distribution problems.

Provides concise, applicable drug information responses and completes lab reviews or pharmacy consults with documentation. Participates in on-call program where applicable and provides experiential education for technicians, students, and new team members. Represents the department in integrating pharmaceutical care across the patient's continuum.

Requirements

  • Bachelor's degree from an ACPE-approved college of Pharmacy (PharmD preferred)

  • Licensed to practice pharmacy in applicable state

Responsibilities

  • Provide accurate medication order-entry, drug distribution, drug administration, diagnostic testing, and patient care
  • Confirm appropriateness of drug therapy for neonates, pediatrics, adolescents, adults, and geriatrics
  • Review and evaluate prescriptions for safety, appropriateness, and clinical efficacy
  • Complete prior authorizations/appeals for medications using EMR data or provider contact
  • Provide initial and ongoing education on medications and create/monitor individual care plans
  • Oversee pharmacy support personnel, check and verify accuracy of tasks performed
  • Provide concise, applicable, timely responses to drug information requests
  • Act as liaison between pharmacy and patient care areas for distribution and supply monitoring
