Join the respected team of medication therapy experts at UW Health as a Clinical Pharmacist specializing in Pediatrics. In this role, you will deliver comprehensive pediatric medication therapy management as part of an interdisciplinary inpatient care team, ensuring drug therapy is safe, effective, efficient, and cost effective.

Your day-to-day work includes actively participating in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review. You will also engage in interdisciplinary patient care rounds, performing medication reconciliation, therapy monitoring, order verification, and individualized drug dosing.

As a teacher and preceptor, you will guide Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education. You will contribute to the advancement of pharmacy practice by developing and implementing clinical practice guidelines, order sets, and standardized protocols, as well as participating in scholarly and quality improvement activities.

Requirements

  • Bachelor's Degree in Pharmacy or a PharmD degree required
  • 1 year of clinical patient care and orders management experience required
  • Licensed to practice pharmacy in the State of Wisconsin within 90 days required
  • Basic Life Support (BLS) within 90 days required
  • Pediatric Advanced Life Support (PALS) within 180 days required
  • Completion of an ASHP Accredited Residency training program preferred
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program preferred
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) preferred

Responsibilities

  • Deliver comprehensive pediatric medication therapy management as part of an interdisciplinary inpatient care team
  • Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review
  • Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education
  • Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient and caregiver education, drug information support, and individualized drug dosing
  • Contribute to the advancement of pharmacy practice through development and implementation of clinical practice guidelines, order sets, and standardized practice protocols
  • Participate in scholarly and quality improvement activities to advance pharmacy practice
  • Provide drug information support and individualized drug dosing for pediatric patients
