VillageMD is a physician-led, patient-centric network simplifying health care through connected services across Village Medical, Village Medical at Home, Summit Health, CityMD, and Starling Physicians. The Ambulatory Care Clinical Pharmacist provides pharmaceutical care using approved protocols, managing patient drug therapy and education while promoting evidence-based, cost-effective care. Independent thinking and decision-making are required in assigned clinical areas.

Daily duties include providing Medication Therapy Management (MTM) under NJ State Board of Pharmacy Collaborative Practice Agreements with primary care providers. Pharmacists promote rational drug therapy by participating in decision-making, initiating adjustments, monitoring regimens via patient interviews, profiles, and records. They optimize outcomes for high-risk conditions like diabetes, hypertension, heart failure, and more through direct and telephonic interactions.

Work collaboratively with health professionals in a compassionate community innovating value-based care. Leverage integrated applications, population insights, and staffing expertise for high-quality services. Focus on cost savings with generics and safe use in patients 65 and older.

Requirements

  • Full pharmacotherapy knowledge including oncology/infusion medications
  • Ability to provide ambulatory pharmaceutical care services using approved protocols
  • Independent thinking and decision-making in clinical areas
  • Experience managing patient drug therapy and providing education
  • Knowledge of collaborative practice protocols for initiating and adjusting therapies
  • Proficiency in optimizing regimens for chronic conditions like diabetes, hypertension, and COPD
  • Skills in patient interviews, monitoring parameters, and drug interaction reviews
  • Familiarity with NJ State Board of Pharmacy regulations

Responsibilities

  • Provide Medication Therapy Management (MTM) services under approved NJ State Board of Pharmacy Collaborative Practice Agreement with primary care providers
  • Promote rational, evidence-based, and cost-effective outpatient drug therapy through participation in drug therapy decision-making with the care team
  • Initiate, adjust, monitor, and recommend changes in drug therapy based on patient interviews, objective parameters, drug profiles, and medical records
  • Optimize medication regimens and clinical outcomes in patients with diabetes, hypertension, heart failure, dyslipidemia, and other high-risk conditions
  • Optimize utilization of cost-saving generic alternatives across all patient encounters
  • Optimize safe medication use by reducing inappropriate medications in patients 65 and older
  • Optimize patient medication adherence through counseling and motivational interviewing
  • Design, implement, and monitor therapeutic drug plans through direct and telephonic interactions with patients and providers
