About this role

Decentralized unit-based Clinical Pharmacists are valued team members working in patient care areas, optimizing medication therapies by monitoring drug selection, dosage, interactions, and reconciliation. They serve as a resource for drug information and education for physicians, nurses, and patients.

They round with Intensivists and Hospitalists, actively recommending therapies and changes. Additionally, they dose medications under protocols, including aminoglycoside, vancomycin, and Coumadin dosing, and write TPN orders for all hospital patients except neonates.

Requirements

Bachelor’s Degree in Pharmacy with 5 years clinical experience OR Doctorate of Pharmacy with completed rotations.
South Carolina pharmacist license required.
Residency trained and board certification preferred.
American Heart Association BLS certification required (must be obtained within 7 days of hire).
ACLS certification required within orientation period.

Responsibilities

Detect problems with drug therapy regimens and recommend suggestions to physicians or nursing staff to resolve drug therapy problems or enhance patient outcomes.
Provide medication instruction to patients and discuss new drug product information with pharmacists, technicians, physicians, and nurses.
Provide aminoglycoside and vancomycin dosing and monitoring for all hospital patients.
Provide Coumadin dosing and monitoring for all consulted patients.
Write TPN orders for all hospital patients except neonates, keeping the physician and dietary apprised of patient needs.
Perform IV to PO conversions, adjust drug dosages per renal function, and execute auto-substitutions per policy.
Round with Intensivists and Hospitalists, actively making recommendations for therapies and changes.
