Clinical Pharmacist - Part Time

The Clinical Pharmacist is responsible for the safe, effective, and fiscally responsible use of medications through the application of clinical knowledge, skills, and professional judgment. This role serves as a patient advocate when planning and implementing pharmacy care by considering each patient's disease states and background.

Day-to-day activities include accurate order verification, dispensing, and minimizing order turnaround time, along with medication monitoring and documentation. The pharmacist also performs documentation of clinical activities and medication reconciliation, ensuring adherence to Hospital Formulary policies and local and system policies.

This position operates within a collaborative healthcare environment at University Hospitals, working alongside other practitioners to uphold the highest standards of patient safety and pharmacy practice. The role is legally responsible for reviewing and approving all activities related to dispensing patient-specific medications pursuant to a valid prescription or medication order.
