About this role

As a Clinical Pharmacist, you will play a critical role in executing our healthcare model to strengthen the provider-patient relationship and engagement.

You will focus on supporting providers and patients through medication management support, pharmacist-led clinical programs, and practice transformation. Your day-to-day work involves optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, reducing preventable ED and hospital utilization, and monitoring adherence to medication regimens and care plans.

You will meet patients where their needs are—virtually, in-office, and at-home—to identify, resolve, and prevent medication-related problems. You will also leverage various technology platforms like Carium and Elation to manage longitudinal care for patients enrolled in your program.

As time allows, you will participate in committees and initiatives such as program development for chronic diseases (COPD, DM, CHF), HEDIS strategies to close gaps, and clinic operations. Requirements

  • Active and unrestricted pharmacist license in the state of Texas
  • Experience in medication therapy management and comprehensive medication reviews
  • Ability to navigate multiple data sources including EMR, pharmacy records, and patient interviews
  • Proficiency with technology platforms such as Carium and Elation for patient care management
  • Strong clinical knowledge in chronic disease states including diabetes, hypertension, COPD, and heart failure
  • Experience working in a primary care or ambulatory care setting
  • Ability to work collaboratively with providers, care teams, and patients in a multi-disciplinary environment

Responsibilities

  • Manage pharmacist consults for chronic care management of certain disease states, post-discharge medication management, and other curbside consults
  • Leverage various technology stack (Carium, Elation, etc.) to manage longitudinal care for patients enrolled in their program
  • Complete comprehensive medication reviews while navigating multiple sources of truth (EMR, pharmacy, patient, etc.) and ultimately creating and following up on a care plan with the patient and other care teams as well as the provider team
  • Efficiently assess prescribed medications to optimize therapy and improve clinical outcomes such as blood pressure and A1c control
  • Reduce preventable emergency department and hospital utilization through medication management interventions
  • Monitor patient adherence to medication regimens and care plans, intervening as needed
  • Participate in committees and initiatives for chronic disease program development, HEDIS strategies, and clinic operations as time allows
