About this role

As a Pharmacist in the NICU at Legacy Health, you are the trusted expert patients and families rely on to understand complex medication therapies and use them safely. You ensure patients are informed and confident in their drug therapy—playing a vital role in their overall care and well-being, especially for the most vulnerable neonates.

In this day-to-day role, you will perform clinical pharmacy services that are age-appropriate for neonates, including therapeutic drug monitoring, drug therapy management, and direct patient consultations. You will administer approved collaborative drug therapy protocols, document medication-related care in the patient's medical record, and monitor for adverse drug reactions.

You will work in close association with health care providers in a supportive, compassionate environment that prioritizes high-quality care and patient outcomes. This position offers the opportunity to specialize in neonatal intensive care pharmacy, participating in drug use evaluations, quality assurance programs, and drug-related research. With PGY1 or PGY2 residency training preferred, this role provides a growth path for pharmacists aiming to deepen their expertise in pediatric and neonatal pharmacotherapy.

Requirements

  • PGY1 Residency Training with pediatric experience or at least 3 years of inpatient pediatric hospital pharmacy experience strongly preferred
  • PGY2 Residency Training and Board Certification preferred
  • Clinical competence in age-appropriate care for neonates, pediatric, adolescent, adult, and geriatric patients
  • Ability to demonstrate clinical competence as required by program needs
  • Skill in therapeutic drug monitoring and evaluating clinical and pharmacokinetic laboratory data
  • Ability to provide age- and educationally appropriate patient education and counseling

Responsibilities

  • Monitor drug therapy for therapeutic appropriateness, duplication, route of administration, and patient compliance
  • Assess drug-drug, drug-food, drug-laboratory, and drug-disease interactions to optimize patient outcomes
  • Provide oral and written consultations to health care practitioners regarding drug therapy selection and management
  • Administer medical-staff approved collaborative drug therapy protocols
  • Develop patient-specific, age-appropriate drug therapy management plans and therapy endpoints
  • Document medication-related care in the patient's medical record and monitor for adverse drug reactions
  • Participate in drug use evaluations, quality assurance, and continuous quality improvement programs
  • Fulfill patient medication orders during assigned shift and delegate responsibilities to prioritize patient care
