This Clinical Pharmacist role optimizes patient care by promoting rational, safe, and cost-effective drug therapy. The position provides essential pharmacist staffing duties and conducts drug therapy monitoring. It implements drug therapy interventions to reduce adverse events and improve patient outcomes.

Day-to-day responsibilities include conducting drug formulary reviews and designing drug use evaluations. The pharmacist collects, analyzes, and trends data to synthesize into reports for formulary evaluations and ongoing monitors. Participation in medical rounds with the interdisciplinary team is a key component.

The role involves educating and training pharmacy staff, Pharmacy Residents, and other healthcare providers. Clinical briefs are prepared for nurses, medical staff, and committees. The pharmacist promotes interdisciplinary teamwork by serving on committees and task forces.

This position offers input into planning pharmaceutical programs aligned with departmental goals. The pharmacist is responsible for the design, coordination, implementation, and outcomes of clinical initiatives. It provides an opportunity to shape drug therapy policies and enhance patient safety.

Requirements

  • Bachelor's degree in Pharmacy or Doctor of Pharmacy (Pharm D).
  • Completion of a Pharmacy Residency or 2 years of demonstrated clinical experience.
  • Licensed or eligible for licensure as a Registered Pharmacist in the state of Pennsylvania (must obtain within 120 days of hire).
  • Preferred: PGY1 Residency.
  • Experience in drug therapy monitoring and formulary management.
  • Ability to design and implement drug use evaluations and synthesize clinical data.
  • Proven ability to educate and train healthcare professionals.

Responsibilities

  • Performs pharmacist functions including drug therapy monitoring, formulary review, and drug therapy interventions to reduce adverse events.
  • Provides clinical support by designing and implementing drug use evaluations, collecting and analyzing data for reports.
  • Participates in educational programs for healthcare professionals including nurses, medical staff, pharmacy residents, and pharmacists.
  • Prepares clear and concise clinical briefs for use by nurses, medical staff, pharmacy, and committees.
  • Participates in medical rounds with the interdisciplinary team to optimize patient care.
  • Provides input into planning pharmaceutical programs to meet department and organizational goals.
  • Promotes interdisciplinary teamwork by working on committees and task forces.
