As a Clinical Pharmacist in Population Health, you will coordinate pharmacy initiatives for registry and high-risk patients identified within LVPHO practices.
In this role, you will critically review drug utilization data, evaluate patient EMR profiles for medication appropriateness, and conduct patient appointments to document drug therapy recommendations. You will identify prescribing patterns that may lead to improper drug selection or adverse reactions and implement strategies to resolve cost barriers.
You will work closely with the Population Health Director and ICC Pharmacy Manager to implement strategic population health pharmacy plans. Your expertise will help positively impact CMS STARS, PQA, and HEDIS quality measures through direct interaction with patients, providers, and pharmacies.
This position offers the opportunity to develop and complete performance improvement projects related to medication use, promoting high-value, safe, and cost-efficient medication therapy management. You will also educate providers and staff through presentations, newsletters, and one-on-one interactions.
Requirements
- Bachelor's Degree in Pharmacy or Doctorate in Pharmacy.
- 2 years of relevant pharmacist experience in patient care or population health, or 1 year of PGY-1 Residency.
- Skilled in critical appraisal of medical literature.
- Capable of independently reaching therapeutic decisions in patient care and communicating them to physicians and other health professionals.
- Ability to accurately monitor medications.
- Ability to make independent judgments under stressful situations.
- Ability to identify, organize, and complete performance improvement projects related to medication management.
- Knowledge of laws, regulations, and accreditation standards related to medication management.
Responsibilities
- Apply subject matter expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence strategies, and Medication Therapy Management to promote positive outcomes in value-based reimbursement contract programs.
- Critically review drug utilization data provided by insurers and analytic team to assure ongoing performance improvement of pharmaceutical care services.
- Evaluate patient EMR profiles for appropriateness of medication therapy, conduct patient appointments, and provide and document drug therapy recommendations.
- Identify prescribing patterns that may lead to improper drug selection, sub-therapeutic dosage, overdose, adverse drug reactions, drug interactions, or medication without indication, and implement strategies to resolve cost barriers.
- Interact with patients, providers, and pharmacy to positively impact CMS STARS, PQA, or HEDIS quality measures.
- Work with Population Health Director and ICC Pharmacy Manager to help implement strategic population health pharmacy plans.
- Provide education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.
- Ensure quality of care through formulary and/or medication management and quality improvement projects.
