About this role

This Clinical Pharmacy Technician role focuses on providing member medication counseling and addressing gaps in treatment by collaborating with healthcare team members and community partners. The technician serves as a Pharmacy Navigator, helping members achieve proper medication adherence and prevent adverse events.

Day-to-day responsibilities include reviewing medication lists, contacting pharmacies to obtain refills, and coordinating with providers to issue new prescriptions or set up appointments. The technician also helps members facing financial issues access their medications and works with community pharmacies to align refills for fewer pharmacy visits.

This role partners closely with pharmacists and the broader healthcare team to communicate medication treatment plans and integrate medication therapy management interventions within overall care. Team members also document care delivered and share information with other providers.

This position offers the opportunity to directly impact patient outcomes by eliminating barriers to therapy and improving medication adherence. The role involves telephonic and possibly in-person communications with members and providers.

Requirements

  • Experience in pharmacy or healthcare setting with medication management duties
  • Ability to review and interpret medication lists and patient profiles
  • Strong collaboration skills to work with pharmacists, providers, and community pharmacies
  • Proficiency in documenting care and communicating treatment plans
  • Skilled in identifying and addressing barriers to medication adherence
  • Comfortable handling member and provider communications telephonically and in person
  • Knowledge of prescription and non-prescription medications, including homeopathic/herbal therapy

Responsibilities

  • Perform review of medication lists to understand and eliminate barriers to medication therapy
  • Facilitate member access to medications by contacting pharmacies for refills and providers for new prescriptions
  • Assist members experiencing financial issues to obtain their medications
  • Work with the member's pharmacy to align refills and reduce frequency of pharmacy visits
  • Partner with pharmacists and healthcare team to communicate and coordinate medication treatment plans
  • Perform patient health assessments to collect medication profiles and identify adherence barriers
  • Document care delivered and communicate information to other providers
  • Handle member and provider communications both telephonically and possibly in person
