L.A. Care Health Plan, the nation's largest publicly operated health plan, was established in 1997 to provide health coverage to low-income Los Angeles County residents. Serving over 2 million members, the organization focuses on ensuring members receive quality care at the right time and place.
The Clinical Pharmacist II will oversee daily operations of the Pharmacy and Formulary department while assisting with research and development of clinical pharmacy programs. This role manages drug utilization, clinical education, and quality improvement activities related to pharmaceuticals.
Day-to-day activities include participating in the Interdisciplinary Care Team, performing medication assessments for members, and supporting Medication Therapy Management initiatives. The pharmacist also provides expertise at Pharmacy Quality of Care meetings and partners with the contracted Pharmacy Benefit Manager on formulary management.
This position involves educating providers on formulary utilization and protocols, as well as representing the Pharmacy Clinical Services Unit at internal and external meetings. The role also includes auditing the PBM, subcontracted plans, and medical groups to ensure continuous monitoring and compliance.

Requirements
- Doctor of Pharmacy (Pharm.D).
- At least 4 years of pharmacy experience OR 1 year of Pharmacy Residency with 1 year of Pharmacy experience.
- Working knowledge of computers.
- Team player with outstanding communication skills (verbal and written).
- Aptitude in problem-solving and decision-making.
- Strong professional ethics.
- Solid knowledge of drug administration and health & safety guidelines.
- Knowledge of the processes and methods to collect, evaluate, prepare and submit information for Adverse Event (AE) reports of post-marketed products.
Responsibilities
- Responsible for the daily operation and decision-making process pertinent to clinical pharmacy programs.
- Provide expertise during Pharmacy Quality of Care (PQOC) meetings and partner with contracted Pharmacy Benefit Manager (PBM) on formulary management requirements.
- Educate providers on the utilization of the formulary and protocols.
- Participate in and provide representation of the Pharmacy Clinical Services Unit at internal and external meetings and workgroups.
- Perform medication assessments for members and for Medication Therapy Management participants.
- Assist in auditing the PBM, subcontracted plans, and medical groups.
- Participate in care management as a member of the Interdisciplinary Care Team.
- Assist with conducting oversight audits and focus on continuous monitoring activities.
