About this role

The Senior Pharmacist - Antimicrobial provides a pharmaceutical service consistent with the mission and ethos of the Pharmacy Department and Beaumont Hospital. The department strives to safely source, acquire, purchase, store, formulate, compound, and distribute drugs to the highest standards. It advises on safe, effective, and rational medicine use to improve patient quality of life.

Review medications for inpatients and outpatients, checking every prescription order for accuracy and suitability per relevant SOP. Ensure safe and efficacious dispensing, obtain detailed patient medication history to manage drug-drug interactions and illicit drug use. Discuss relevant interactions with consultants, educate patients on medications, and manage side effects according to procedure.

Participate in the pharmaceutical service as part of a punctual, dedicated, and professional team. Demonstrate behaviour consistent with Hospital Values, create open communications, and maintain healthy working relationships. Subject to direction from Chief II Pharmacist and Head of Pharmacy Services, upholding equity, accountability, and quality.

Identify, develop, and implement policies and operational processes to best practice standards under supervision. Participate in departmental mentor/mentee and tutor/tutee programmes. Advise medical and nursing colleagues on safe, effective medicine use and provide Medicines Information.

Requirements

  • Registered Pharmacist with the Pharmaceutical Society of Ireland
  • Knowledge of antimicrobial stewardship and rational antimicrobial use
  • Clinical pharmacy practice in hospital settings
  • Expertise in medication review and therapeutic drug monitoring
  • Ability to assess prescriptions for antimicrobial appropriateness and interactions
  • Proficiency in interpreting laboratory results relevant to antimicrobial therapy
  • Experience providing medicines information and clinical advice

Responsibilities

  • Maintain throughout the Hospital an awareness of the primacy of the patient in relation to all activities
  • Participate in the pharmaceutical service as part of a punctual, dedicated and professional team
  • Review medications for inpatients, checking every prescription for accuracy and suitability per SOP
  • Obtain detailed patient medication history to manage drug-drug interactions and discuss with consultants
  • Educate and counsel patients about medications and manage side effects according to procedure
  • Review relevant laboratory results and monitor adherence to treatment
  • Advise medical and nursing colleagues on safe and effective use of medicines
  • Identify, develop and implement policies and processes under supervision to best practice standards
