At Hill Physicians Medical Group, support the development and implementation of programs, policies, and procedures that promote medication optimization and high-quality disease state programs. Conduct outreach to patients for medication management in chronic disease states and collaborate with physicians for longitudinal patient management per protocol. Perform comprehensive medication reviews for high-risk, polypharmacy, or non-adherent patients, communicating recommendations to primary care physicians and specialists. Participate in virtual care team rounds and post-discharge medication reconciliation to reinforce follow-up care.
Collaborate with the Quality team on interventions ensuring high IHA and CMS Star quality measure compliance, especially in medication and disease state management. Perform patient and provider outreaches to close quality gaps and flag generic drug switch opportunities. Evaluate specialty drug regimens for optimization recommendations to physicians.

Requirements
- Pharm.D., with Completion of a PGY-1/Residency program preferred or equivalent practice experience
- Valid current California Pharmacy license required
- Advanced Practice Pharmacist license preferred
- Experience working with chronic disease populations (e.g., Diabetes, hypertension) preferred
- Excellent attention to detail, organizational skills, and ability to manage various projects at the same time
- Self-directed
- Excellent interpersonal, verbal, and written communication skills
- Computer skills and working knowledge of Microsoft Office applications; ability to learn new clinical systems
