About this role

Washington Regional Medical System is seeking a Clinical Pharmacist to join our pharmacy team in Fayetteville, Arkansas. This role reports to the Director of Pharmacy and is responsible for providing safe, effective, evidence-based medication therapy management in collaboration with physicians, nurses, and other interdisciplinary care partners.

In this evening shift position with weekend rotation, you will review, verify, and monitor medication orders while evaluating drug therapy for appropriateness, safety, efficacy, interactions, and patient-specific clinical factors. You will serve as a medication therapy expert and resource to clinical teams, patients, and caregivers by providing drug information, patient education, and medication reconciliation support.

This position offers the opportunity to support day-to-day pharmacy operations, medication safety initiatives, quality improvement activities, and the supervision of pharmacy support personnel and students. Requirements

  • Active Arkansas Pharmacy License
  • Ability to evaluate drug therapy for appropriateness, safety, efficacy, interactions, allergies, contraindications, and patient-specific clinical factors
  • Knowledge of medication order review, verification, and processing in accordance with organizational policies and regulatory standards
  • Experience providing drug information and clinical consultation to healthcare providers and patients
  • Proficiency in medication reconciliation, patient education, and monitoring for adverse drug events
  • Ability to prepare and dispense medications including controlled substances and sterile/non-sterile preparations
  • Strong collaboration skills to work effectively with physicians, nurses, and interdisciplinary care partners
  • Capability to supervise and guide pharmacy support personnel and students

Responsibilities

  • Review, verify, and process medication orders to ensure accuracy, completeness, appropriateness, safety, and compliance with organizational policies and state/federal regulations
  • Evaluate patient-specific medication therapy including indication, dosing, route, frequency, allergies, renal and hepatic function, laboratory values, drug interactions, and therapeutic duplication
  • Monitor patient drug regimens and collaborate with physicians, nurses, and interdisciplinary team members to promote safe, effective, evidence-based medication use
  • Provide timely drug information, clinical consultation, and medication-related recommendations to providers, nursing staff, patients, and caregivers
  • Perform medication histories, medication reconciliation activities, and patient education on medication purpose, administration, adherence, and potential adverse effects
  • Prepare, dispense, and oversee the distribution of medications including controlled substances, sterile and non-sterile preparations, in accordance with professional standards
  • Support medication safety practices by identifying, documenting, and helping resolve medication-related concerns, near misses, and adverse drug events
  • Supervise and guide pharmacy support personnel and students in daily operations
