About this role

This full-time position follows a 7-on/7-off rotation with 10-hour evening shifts from 12-10pm, providing specialized care to cardiac patients. Candidates should be comfortable cross-training to cover emergency department pharmacist shifts as a secondary work location.

Day-to-day responsibilities include attending patient care rounds, processing medication orders, and performing patient-specific medication profile reviews. The pharmacist executes protocol-based drug therapy such as pharmacokinetic dosing and IV-to-PO conversions, adjusting doses for renal or hepatic dysfunction.

As part of a multidisciplinary team, this role supports pharmacy residency programs by precepting rotations and providing constructive feedback. The pharmacist also shares expertise through educational presentations and in-services for pharmacy and non-pharmacy staff.

Opportunities include flexing to other areas or alternative scheduling to assist with supplemental shifts. The role actively participates in performance improvement efforts to strengthen systems and processes that lead to improved pharmaceutical care.

Requirements

  • Bachelor's Degree in Pharmacy from an accredited college or university
  • Licensed Pharmacist - State Board of Pharmacy in state of primary practice site
  • Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) certification as necessary
  • Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
  • Completion of an accredited pharmacy post graduate training program (PGY-1 and/or PGY-2 residency program) preferred
  • Board of Pharmaceutical Specialties certification in chosen area of specialty preferred

Responsibilities

  • Actively participates on the multidisciplinary team by consistently attending patient care rounds and providing patient-focused pharmaceutical care
  • Assures timely medication order processing by participating in order entry, order verification, and reviewing orders for timely resolution of problem orders
  • Completes patient-specific medication profile reviews and executes protocol-based drug therapy including pharmacokinetic dosing and IV-to-PO conversions
  • Participates in medication reconciliation at the time of admission, transfer, and/or discharge and provides patient education when appropriate
  • Shares pharmaceutical expertise by coordinating and providing educational presentations or in-services for pharmacy and non-pharmacy staff
  • Supports pharmacy residency programs by precepting organized rotations, providing timely feedback, and collaborating on presentations and projects
  • Actively supports department staffing needs by flexing to other areas or alternative scheduling to assist in coverage of supplemental shifts
  • Reports medication incidents and adverse drug reactions in Patient Safety Net (PSN) and participates in medication incident review or peer review activities
