Clinical Pharmacist - Infectious Diseases/Antimicrobial Stewardship

The Infectious Diseases Clinical Pharmacist supports and advances Antimicrobial Stewardship Program initiatives across the Spaulding Rehabilitation Network and island hospitals including Martha's Vineyard Hospital and Nantucket Cottage Hospital. The position combines direct clinical support with system-focused stewardship efforts across acute rehabilitation and critical access hospital sites.

This pharmacist collaborates closely with interdisciplinary clinical teams to optimize antimicrobial therapy and support evidence-based prescribing practices. Key responsibilities include prospective audit and feedback, interpretation of microbiologic results, and providing patient-centered recommendations for antimicrobial regimen optimization.

The role promotes safe and effective use of anti-infective agents while minimizing antimicrobial resistance, medication-related toxicities, and other unintended patient harms. The pharmacist attends daily medical and multi-disciplinary rounds and participates in specialty rounds as needed.

Additional responsibilities include development and implementation of clinical guidelines and workflows. The position also supports regulatory compliance, accreditation readiness, and quality improvement initiatives across the network.
