About this role

The Pharmacist provides quality pharmaceutical care, assuring safe, efficacious, and cost-effective medication therapy for all patients. Develops, recommends, and monitors evidence-based pharmacotherapy care plans with emphasis on patient outcomes and quality of life. Includes assessment of medications for indication, effectiveness, safety, adherence, plus monitoring and documentation.

Assumes responsibility for pharmaceutical care delivery in assigned patient care area, coordinating resources for timely, efficient care. Receives and interprets physicians' medication orders, clarifying or correcting as needed. Reviews patient profiles to guard against sensitivities, contraindications, duplications, and drug interactions.

Consults and advises medical staff on drug therapy problems, monitoring lab results and providing in-depth drug information to healthcare professionals. Makes rounds with medical services and supervises pharmacy technicians, students, and interns. Coordinates activities with pharmacy, medical, and nursing personnel to resolve issues.

Calculates drug dosages using pharmacokinetic parameters and prepares special medications with aseptic techniques. Provides patient discharge consultations and in-services to healthcare providers. Assists with departmental policies and ensures compliance with standards in clinical environment.

Requirements

  • Bachelor's Degree-Pharmacy (Required)
  • Doctoral Degree- (Preferred)
  • Ability to review medication orders for prescribing, formulary, and therapeutic issues
  • Knowledge of pharmacokinetic parameters for drug dosage calculations
  • Proficiency in aseptic techniques for compounding sterile products
  • Skill in monitoring patient laboratory results and outcomes
  • Experience interpreting physicians' orders and patient profiles

Responsibilities

  • Assumes responsibility for the delivery of pharmaceutical care to the specific patient care area
  • Receives and interprets physicians' medication orders and clarifies or corrects orders with the physician
  • Reviews patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions
  • Suggests appropriate formulary alternative(s) based on the patient's clinical parameters
  • Consults and advises members of the medical staff on matters relating to specific drug therapy problems
  • Makes rounds with medical services
  • Calculates drug dosages based on pharmacokinetic parameters
  • Advises, assists, and supervises the work of pharmacy technicians, students, and pharmacy interns
