About this role

This role oversees the provision of safe and effective medication therapies through collaboration with the patient's care team. The clinical pharmacist reviews physician orders for drug therapy and monitors for drug choice, dosage, interactions, adverse reactions, and formulary adherence.

Day-to-day responsibilities include retrieving and evaluating biomedical literature to provide concise drug information responses, counseling patients and caregivers on medication therapy, and ensuring accurate shift-to-shift communication. The pharmacist also designs, recommends, monitors, and evaluates patient-specific pharmacotherapeutic regimens, documenting interventions appropriately.

Working primarily in the pharmacy, the role involves delivering medications to nursing units and participating in the management of medical emergencies. The pharmacist precepts pharmacy students and residents while preparing and dispensing medications in accordance with best practices and regulations.

Requirements

Bachelor's Degree in Pharmacy or a Pharm.D. from an accredited school of pharmacy required.
Pharmacist license issued by the state in which the team member practices required.
Previous hospital pharmacy experience preferred.
Immunization Certification required (Medical Center Rx only).
Highly effective verbal communication and interpersonal skills with patients, visitors, and other healthcare team members to establish working relationships that foster optimal quality of patient care.

Responsibilities

Oversee the provision of safe and effective medication therapies through collaboration with the patient's care team.
Review physician orders for drug therapy and monitor for drug choice, dosage, interactions, adverse drug reactions, and formulary adherence.
Retrieve and evaluate relevant biomedical literature, formulate appropriate responses, and provide timely drug information to nurses, physicians, and other healthcare workers.
Use effective patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and administration.
Design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens and document interventions using clinical intervention policies and procedures.
Participate in the management of medical emergencies and precept pharmacy students and residents.
Prepare and dispense medications in accordance with best practice, policies, and state and federal regulations, and check the accuracy of work performed by pharmacy support team members.
