Requirements

Doctor of Pharmacy (PharmD) degree from an accredited institution
Active pharmacist licensure in the state or region of practice
Advanced training or certification in population health management or a related field (preferred)
Strong analytical and data interpretation skills
Experience in pharmacy practice, preferably in a population health or managed care setting
Excellent communication, collaboration, and leadership abilities
Knowledge of healthcare regulations, policies, and quality improvement methodologies

Responsibilities

Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes
Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related outcomes
Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring per population-specific needs and evidence-based guidelines
Design and implement programs to educate patients about their medications, ensuring understanding, adherence, and management of potential side effects
Work closely with healthcare teams, including physicians, nurses, and social workers, to coordinate care plans and interventions
Participate in quality improvement projects to enhance medication-related outcomes, reduce adverse events, and promote evidence-based practices
Stay updated with pharmaceutical regulations, ensuring compliance with relevant laws, policies, and guidelines
Provide leadership within the pharmacy team, mentorship to junior staff, and contribute to departmental goals
