About this role
The Ambulatory Care Clinical Pharmacist promotes rational, safe, and cost-effective drug therapy in collaboration with primary care providers, specialists, nurse care managers, and community health partners. This role focuses on identifying and resolving medication therapy opportunities while co-designing individualized therapeutic regimens for optimal chronic disease management and preventive care.
Day-to-day responsibilities include providing Comprehensive Medication Management (CMM) directly to members and their caregivers, developing medication management action plans, and conducting ongoing assessments to refine care plans. The pharmacist communicates with internal and external providers to ensure alignment with medication-related goals and supports clinical team members with drug information, dosing, and interaction questions.
The pharmacist works within a hub-based and virtual clinical care team, collaborating with both clinical and non-clinical roles to drive better individual and population health outcomes. This environment emphasizes team-based care, with eventual opportunities to transition to collaborative practice agreements (CPA) with Cityblock physicians, allowing for member assessments, lab orders, and medication adjustments per protocol.
This role offers growth through participation in staff education, development of clinical pathways and protocols, and training team members on medication management tools. The position supports professional development in a remote setting with company-sponsored cross-licensure opportunities and a focus on reducing avoidable emergency department visits and hospitalizations.
Requirements
- Active pharmacist license in at least one of the following U.S. states: Michigan, Illinois, or Ohio.
- Eligibility and willingness to obtain Ohio licensure upon hire (company-supported and reimbursed).
- PGY1 pharmacy practice residency and a PGY-2 Ambulatory Care Pharmacy Residency, or a minimum of 2 years’ experience as a clinical pharmacist in an ambulatory care setting.
- Strongly preferred: Active Ohio pharmacist license at time of hire and additional licensure in Michigan and/or Illinois (or willingness to pursue cross-licensure post-hire).
Responsibilities
- Provide Comprehensive Medication Management (CMM) in collaboration with hub-based and virtual clinical care teams and other pharmacist support services as needed.
- Contribute to the development of a medication management action plan to achieve member self-management goals and clinical outcomes with measurable parameters and timeline for monitoring and follow-up.
- Communicate and work collaboratively with Cityblock and external providers to ensure all clinical interventions align with medication-related goals, conducting ongoing assessments and refinements to the care plan.
- Provide direct support to clinical team members for general drug information, dosing, interaction/adverse effect avoidance questions, or member-specific needs.
- Participate in staff education through presentation of medication management in-services, development of educational tools, and training/coaching on maintaining a member’s best possible medication list and post-discharge med reconciliation.
- Help develop medication management and disease state-specific clinical pathways/protocols, working toward collaborative practice agreements with Cityblock physicians.
