About this role
This Ambulatory Care Clinical Pharmacist role, focused on high cost drugs, is employed by the Greater Rochester Independent Practice Association (GRIPA). GRIPA is a unique collaboration between hospitals and physicians dedicated to improving both the quality and efficiency of healthcare. The position is based at 100 Kings Highway South in Rochester, NY.
The pharmacist will design programs targeting high medication costs and will directly outreach to providers and patients to educate and support transitions from high-cost medications to lower-cost alternatives of similar efficacy and safety. This role also involves supporting outcome reporting related to medication cost initiatives and coordinating care with contracted entities, including insurers and employer groups.
Day-to-day work includes providing drug information and pharmacy resources to physicians and staff, participating in face-to-face patient encounters in homes or provider offices, and traveling or teleconferencing with care teams across at least 10 counties. The pharmacist will learn and gain proficiency in applications used for high cost drug and care management programs.
This full-time, days shift position offers an opportunity to drive positive outcomes in a collaborative, team-based environment. The pharmacist will work closely with key health system entities and healthcare professionals to foster a team approach to patient care, with a focus on rational and cost-effective prescribing.
Requirements
- Doctorate degree in pharmacy preferred or equivalent combination of education and clinical pharmacy experience
- 3+ years of clinical long-term care, managed care, and/or ambulatory clinical pharmacist experience and/or pharmacy residency
- Experience managing formularies and high cost medications
- Pharmacy license in New York State
- Board certification in Pharmacotherapy, Ambulatory Care, and/or Geriatrics preferred
- Bilingual skills preferred
Responsibilities

- Travel to or teleconference with care team or providers across at least 10 counties
Benefits
- Pay range: $115,500.00 - $162,000.00
- Full-time, days schedule with 40 hours per week
