Cardinal Health's Health System Pharmacy delivers customized solutions that reduce costs and enhance patient care quality for hospitals and integrated providers. We manage the pharmacy at PAM Health Rehabilitation Hospital of Westminster. Seeking a PRN Pharmacist for as-needed shifts during facility hours and on-call coverage.
Pharmacists deliver pharmaceutical care including oversight, monitoring, preparation, dispensing, and documentation of patient medication therapy. Provide drug information, patient counseling, and oversight of dispensing systems. Ensure compliance with Federal, State, and local regulations plus proper pharmaceutical storage.
Work collaboratively with the pharmacy team in a rehabilitation hospital setting open Monday-Friday 8am-4:30pm. Handle on-call duties after hours, weekends, and holidays as needed. Participate in quality improvement, medication reviews, and staff education programs.

Requirements
- Bachelor's degree in Pharmacy or PharmD
- Licensed pharmacist in good standing in the state of Colorado
- In-patient hospital pharmacy experience preferred
- Flexibility to work various shifts
- Ability to work on-call or in an on-call rotation
- Ability to hear, write, and speak clearly to communicate with customers and health care professionals
Responsibilities
- Interpret medical orders and transcribe to computerized patient medication profiles accurately
- Compound and dispense pharmaceuticals including sterile, chemotherapy, and parenteral nutrition products
- Issue controlled substances to patient care areas and maintain required records
- Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness
- Assist with pharmacokinetics consult service and drug dosing per protocol
- Review culture and susceptibility data for antibiotic appropriateness and recommend changes
- Detect and report suspected adverse drug reactions accurately and timely
- Provide clinical consultation to practitioners and suggest cost-effective alternatives
