About this role
The primary responsibilities of the clinical pharmacist include providing comprehensive pharmaceutical care to all patients within Baystate Health. Under the direction of the Pharmacy Manager, duties encompass age appropriate clinical pharmacy services, evaluation of medication orders for appropriateness, monitoring, patient follow up, drug information consultations, and support of pharmacy department operations.
Maintain competency, training, and ongoing education in pharmaceutical knowledge, integrated technology, and practice model changes. Complete and document tasks like Clinical Interventions and Safety Reporting Events in an electronic database. Adhere to departmental, organizational policies, procedures, laws, and regulations for pharmacy practice management.
Effectively utilize the Clinical Information System (CIS) - Cerner to evaluate orders for drug, strength, dose, allergies, interactions, duplication, stability, compatibility, and formulary status. Perform clinical duties including IV to PO dosing and Antimicrobial stewardship programs. Maintain accurate documentation of clinical interventions and notes, meeting departmental standards for order verification rate.
As an integrated Pharmacy Team member, actively participate in working committees, educational activities, and precept IPPE/APPE students. Maintain competency in core inpatient areas like clinical order verification, drug information, USP 797 compounding, and qualified satellite areas such as Chemotherapy Pharmacy or NICU.
Requirements
- Competency in clinical order verification and drug information using CIS - Cerner
- Knowledge of USP 797 compounding and inpatient pharmacy core areas
- Proficiency with pharmacy technologies including Baxa, TPN compounder, Pyxis med stations, CII safe
- Ability to document Clinical Interventions and Safety Reporting Events in electronic database
- Understanding of distributive services delivery modes like pneumatic tube and grey bags
- Qualified for satellite areas such as Chemotherapy Pharmacy or NICU
- Adherence to departmental standards for safe order verification rate
- Experience precepting Introductory Pharmacy Practice Experience (IPPE)/Advanced Pharmacy Practice Experience (APPE) students
